We the parents/legal guardians of the applicant hereby submit an application for admission to American Institute Allied With Thornton Academy, in accordance with the terms and philosophy stated on the school website, which we have read, and with which we agree. We authorize American Institute Allied With Thornton Academy to contact schools and other sources to obtain information to support this application. The undersigned releases every person and institution from any liability resulting from, or pertaining to, the furnishing of records, documents, and other information provided to American Institute Allied With Thornton Academy for that purpose.

We, the undersigned, hereby submit that, to the best of our knowledge, the information contained in this application is true and accurate. As such, we authorize the Admissions Department to verify any part of the application material. We understand that information regarding the applicant’s personal, educational, medical or disciplinary history which is incorrect, incomplete or withheld may affect admission to the school, including the reversal of a decision for acceptance.
